Mine Information:  Please complete the following information pertaining to your mine.  (You can make any additional comments in the box provided at the bottom of the page)

Equipment Information:  Scroll through the form or use the list below to jump to a particular type of equipment.  Next to each model, please indicate the number (in the # field) of those machines that you have at your mine (leave the field blank for models which you do not have-you DO NOT have to enter a 0).  Throughout the form, space is provided to list any models or different OEM machines that you may have, which are not listed (Please type the entire model number (or manufacturer & model) ).  You can make any additional comments in the box provided at the bottom of the page.  Thank you again for your help and we appreciate any feedback.
